Prior to Scheduling~
To Schedule an Appointment~
- Search calendar for first available date which is generally about 1.5 months out.
- Schedule the correct appointment regarding 'Complex case if taking medication'.
- The appointment type will be changed and charged accordingly.
- The appointment is charged at the time of scheduling.
A minimum of 2 days by 9am CST to reschedule or cancel is necessary to avoid forfeit of total payment.
- If appointment is cancelled or rescheduled 3 days after 9am CST, only 1/2 of payment will be refunded. If rescheduled 3 days after 9am CST 1/2 of the appointment fee will be charged at the current appointment time (2 days prior by 9 am CST), and full payment will be charged at the new appointment time.
An 'appointment reminder' email notification is sent 4 Days 9am CST prior to the appointment.
If there is a opening in the schedule, New Patients who have completed their QUESTIONNAIRE and have uploaded current lab results if available and or applicable will be moved forward if they so choose.
Request in the message area while making appointment that you would like to move forward if available.

After Appointment is Scheduled~
- Check your spam folder if you do not see an email notification from CHARM PHR.
- Set up your HIPAA compliant patient portal-Charm PHR.
- Complete the Health Questionnaire under Questionnaire tab.
- Complete the Questionnaire minimal TWO DAYS by 9am CST prior to appointment. (an email notification appointment reminder is sent 3 days 9am CST prior to appointment day).
- UPLOAD to the patient portal to the DOCUMENT FOLDER (not the lab folder) any lab results you may have in the past year (no medical records) and SHARE with CARE PROVIDER (Dr. Icenhower).
- Your phone number on file in your patient portal will be called at your appointment time.
- Set aside the time as if you were in office - no multi-tasking and or driving.
The scheduler is HIPAA compliant and a Certified Secure portal which is separate from this website via CHARM EHR and Bluefin Payment Gateway – compliant with the PCI-DSS (Payment Card Industry -Data Security Standard).
